DR. HOWARD L. PRANIKOFF LEADERSHIP/EDUCATIONAL MEMORIAL FUND

Background:
Dr. Howard Pranikoff was an ardent supporter of organized dentistry and held many leadership positions throughout his years as an American Dental Association, Florida Dental Association, and Central Florida District Dental Association (CFDDA) member.

The CFDDA remembers and honors Dr. Pranikoff by establishing the Dr. Howard L. Pranikoff Leadership/Educational Memorial Fund was established to provide leadership training to young members.

Application Information:
This grant application is meant to assist dentists who have a passion to lead and be active in the tripartite (ADA, FDA, CFDDA) by attending Leaders Emerging Among Dentistry (LEAD), the FDA's Dentists' Day on the Hill (DDOH), or other leadership development event approved by the review committee.

Any first-time attendee is encouraged to apply by completing this application form and submitting a letter to the Central Florida District Dental Association. The grant can be used towards transportation, lodging, and registration fees. Reimbursements are limited to $500.00 per applicant, with preference given to CFDDA Members. A brief summary of your experience post-event is also required to assist the CFDDA in promoting this endeavor and to assist in raising funds.

Applications will be reviewed by the CFDDA Howard Pranikoff Memorial Scholarship Committee (HLMPS). Reimbursement may occur upon review of submitted documents by the HLMPS Committee and all receipts and summation of your experience at the event to the CFDDA. It is recommended members wishing to receive reimbursement consult with the HLMPS Committee (via cfdental@cfdda.org) prior to their travel and enrollment to be confident the course will apply to the leadership goals of the scholarship fund.
